By submitting the Direct Debit mandate form you must read and agree to the following statements.
I am happy for my information to be used to perform a credit check.
I can afford to repay the monthly amounts previously mentioned.
I am happy to enter into a credit agreement with our finance provider.
I understand that if I fail to pay an instalment on time, I may be charged a default fee.
I understand that missing payments may also affect my credit rating.
I understand that if I fail to make any payments due, my policy/policies may be cancelled and the credit agreement with the finance provider terminated.
